NOTICE: Evaluation Fee Acknowledgement

$125 evaluation fee will be charged if the leak standard can not be calibrated or repaired, other calibration charges will be waived/refunded (LSLE01)

NOTICE: Repair Service Acknowledgement

I agree to an additional $215 repair fee if the leak standard can not be calibrated due to contamination. After repair, the rebuilt leak standard will be calibrated. Only applies to LACO manufactured leak standards. (LSLR01)

Biological Hazard Acknowledgement

I verify the return material shipment is free of biological hazards, radioactive material, organic metals or mercury contaminated chemicals. This is in compliance with Federal OSHA Safety Standard 1910-1200, Hazard Communications “Right to Know”, to ensure LACO personnel safety when receiving, disassembling, or servicing potentially contaminated equipment.
